We work with social workers, care coordinators, mental health teams, local authorities, GPs, housing providers and other professionals.
Our aim is to provide a clear and collaborative referral process so that we can understand whether Community 360 is the right service for the individual.
We also welcome enquiries from other professionals involved in an individual’s care and support.
OUR REFERRAL PROCESS
We understand that finding the right supported living service is an important decision. Our referral process is designed to be straightforward, collaborative and focused on understanding the individual.
Initial Enquiry
Contact our team to discuss the individual and their support needs.
Information Review
We review the available information to gain an understanding of the person's needs and circumstances.
Assessment
Where appropriate, we meet with the individual to understand their needs, preferences, strengths and goals.
Planning the Next Steps
If Community 360 is suitable, we work with the individual and relevant professionals to develop a robust care plan and a clear, person-centred transition plan.
Our focus throughout the process is to understand the individual and ensure that we can provide the right environment and support for their needs.
If you are supporting someone who may benefit from Community 360, contact our team to discuss their needs and find out whether our service could be suitable.
